I, Full Name (required): am participating voluntarily in a swim program managed by the Aurora Master Ducks who are an affiliated member of Swim Ontario.
I hereby acknowledge that I am aware of and hereby accept the risks, dangers and hazards inherent and associated with or related to the sport of swimming, including any injury or health-related problem, which can be severe and even fatal. The risks may include, but are not limited to, possible exposure to a communicable disease, such as Covid-19.
I understand and agree that the Aurora Master Ducks do not provide insurance to cover expenses related to Covid-19 and other contagious diseases.
I fully release and discharge the Aurora Master Ducks and it’s employees, officers and agents from all liability in connection with my participation in club activities.
